In order to submit your Proposal to The Local Government Purchasing Cooperative you must follow the below instructions. Each form listed below must be:

  1. Printed 
  2. Completely filled out
  3. Returned to the Purchasing Cooperative no later than October 14, 2008 at 3:30 PM. Completed Proposals must be mailed in a sealed envelope or hand delivered in a sealed envelope to:

The Product Specification form is available as an Excel Worksheet.  The Cooperative highly suggests that all Product Specification forms be completed in Excel, saved to diskette and returned with a printed copy and all other required forms. Returning your Product Specification form on diskette will help ensure the accuracy of the Proposal Tabulation. In the event that the paper copy and diskette have non-matching data, the data provided on the paper copy will prevail.
